While the phone and fax will remain important communication tools for assigning claims, we have developed an easy method that lets your keyboard do the talking. Our online claim assignment form allows you to submit a claim electronically. Simply fill in the claim information, press the Submit button, and your claim assignment is immediately delivered to our Central Processing Unit. We'll also send you an email verifying receipt of the claim for your assurance.

Step 1: Click on a button above to either use the full online ‘Assign a Claim' form, or use the shorter version if you are submitting an attachment. An online claim assignment form will appear.
Step 2: Complete the claim assignment form and click the Submit button located at the bottom of the form. A confirmation page will appear so that you may review the information that you have entered.  If the information is correct, click the Submit button at the bottom of this page to submit your claim assignment.  You will then receive an immediate email confirming your submission.
