Online claim assignment made easy

The phone and fax will remain important communication tools for assigning claims. We have developed an easy method that lets your keyboard do the talking. Our online claim assignment form allows you to submit a claim online. Simply fill in the claim information, hit the Submit button, and your claim assignment is immediately delivered to the office of your choice. We'll also send you an email verifying receipt of the claim for your assurance.

Instructions for online claim assignment

Step 1: Click on the ‘Assign a Claim' button located above. An online claim assignment form will appear.
Step 2: Complete the claim assignment form and hit the ‘Submit' button located at the bottom of the form. A confirmation will appear assuring you that the claim has been sent. You will receive an immediate email confirming your submission.
