Step 1: Click on either button located above. An online claim assignment form will appear

Step 2: Complete the claim assignment form and include any attachments

Step 3: Hit the ‘Submit’ button located at the bottom of the form.
A confirmation will appear assuring you that the claim has been sent. You will receive an immediate email confirming your submission.
